Membership Cancellation Request Form & Survey


We understand that schedules can change, and we’re happy to help with adjustments whenever possible.

As per the terms of service you agreed to upon your enrollment:

  • To cancel ongoing lessons, please submit this form by the 15th of the month prior to your intended cancellation date.

  • This allows us enough time to adjust schedules and offer the lesson time to other students.

  • If notice is received after the 15th, the following month’s tuition will still be processed, as your lesson time has already been reserved. Tuition is non-refundable once billed.

    • If you don’t give notice by the 15th the tuition for the following month will still be processed, but the credits will be held until your return so your first month back will be paid in advance.

    • Any lesson credits remaining in your account will eventually expire. Optionally, you can DONATE these to our scholarship program for financially challenged families, or you can GIFT them to a friend or family member.

Please note that notifying your teacher directly or sending a text message does not count as official notice — all cancellations must be submitted via email to ensure they are properly recorded.
